Andrew Form Biography
Andrew Form (born February 3, 1969) is an American film and television producer. He is the co-founder of Platinum Dunes with Michael Bay and Brad Fuller and Fully Formed Entertainment with Fuller. He is currently a producer at Sunday Night Productions.
